Can you get there by fast traveling from the War Table? I've had instances where either the pyramidal map marker on the War Table map has disappeared or the round flat button on the fast travel map has disappeared because one or the other did not load completely, but never both. Heck, the Temple of Dirthamen is still there, and I cleared that out ages ago.
Nope it has completely disappeared from my map and the war table. I had to double check with another completed game, but yes. It's gone.

OK, my fully-patched post-epilogue save shows the Cradle of Sulevin in its usual place slightly southeast of the Emerald Graves. I've completed the quest for the Sulevin Blade, FWIW.
I've completed it too, but I can't figure out why it's no longer accessible. That's the one mosaic piece I'm missing. I'll have to go back through my saves and see if I can figure out when it vanished. Thanks for trying to help!
Um... there's no mosaic in the Cradle of Sulevin. Are you thinking of the Knight's Tomb? Because that's accessible by a fast travel point in the Emerald Graves, not by a stand-alone map marker.
One piece of the Freed Are Slaves Mosaic Collection is in the Cradle of Sulevin...
Some of the items (such as the mosaic piece or the spirit rune) in CoS are easily missed as they are in dark alcoves.
Oh yeah, that's right, there's one in the Starlight Chambers. I usually just grab it on my first trip and then forget about it.

I am having the exact same issue. Sulevin is completely missing from Table and Map. I can confirm that I still have it on my first play through, so wondering why the 2nd play through doesn't have the map marker any longer.
Mine is still missing. Let me know if yours ever returns and if you did any thing special to make it reappear.
