I have a char I pushed through hard core (without any problems, really) starting at level one. I am thinking about going for the insanity achievement since its the only thing I don't have (besides the Pinnacle Station stuff). Can I do a NG+ with this char to get this achievement? I ask because when it starts out the game it defaults the difficulty to the last setting used: hard core. I am afraid that bumping up the difficulty after the intro will make it so I don't get the achievement.

I've never done this (only went as far as Hardcore for ME1 too), but this should help:

Apparently if you change the difficulty before leaving the Normandy, it won't matter. I might test it myself soon, so if I finish it before you do, I'll tell ya.

I should add that when I did it, I changed it to insanity when I chose the same character before it showed Shepard looking out the window. I also just did the main quests and nothing else. I got the trophy a short time later. Very easy.

Yes, just make sure the difficulty is set to Insanity before the end of Eden Prime (examining the beacon).

No, one should change the difficulty right after the conversation with Joker, Kaidan and Anderson (radio), when Anderson asks Shepard to meet him and Nihlus in the comm room. When you touch down on Eden Prime, it's too late to change the setting, so for that reason, don't start the conversation with Nihlus before changing the setting.

Are you sure?

I got the 'hardcore' trophy after I switched to that difficulty half-way during Eden Prime, so I'm confident that Capn is correct.

It has been many years since I got this achievement, and I did it with a NG+ character, so I don't remember exactly when I changed the difficulty.  But all the topics I had seen on it said before the beacon.

If you want to set it on Normandy to be sure, go ahead.  At least you won't be using the slight exploit people reported who ran through Eden Prime on Casual and then bumped it up.

Quote from Playstationtrophies.org's guide:

19B.jpg Medal of Valor trophy_gold.png Complete 1 Mass Effect playthrough on the Insanity difficulty setting. Do not change the setting.

Insanity is unlocked after you complete a full playthrough on Hardcore. As a result, this trophy will unlock after your third playthrough. Just be sure that you change the difficulty before you leave the Normandy, just to be safe. You also have to save the changes or else they won't stick.

Insanity isn't too much harder than Hardcore. The enemies are tougher and do more damage though. You will be a pretty high level coming in, so Insanity won't be too bad."
