2. You've already said yourself how difficult that would be, no? Due to the distance that would need be traveled, or scoured over, just to find one such place? There were other, higher priorities.

4. I would say that fairly incorrect. Lelianna wishes you to ivestigate because she is worried. Why is she worried? Because she thinks somethings wrong. She also thinks the Wardens may be involved and wants you to find evidence to the contrary. Blackwall's dialogue when you first meet him and tell him about the Wardens - to paraphrase off the top of my head, he says he understands why you'd be suspicious of them but that Wardens would never be involved with something like the Breach. The Wardens being missing would imply something was wrong all on its own, really.
