Guard is 25% of you max HP.
If you have 500 HP, that means Guard is equivalent to 125 HP.
The first time you hit with Dwarven Longsword, you gain 3 Guard HP, which puts you at 3/125. This shows immediately on the HUD because it must show you now have more than 0 Guard.
Hit again, you're at 6 Guard HP (3 + 3 = 6), but the HUD doesn't have to update because each "Guard bar" represent a range like 25 Guard HP so it stays the same on the HUD.
When you'll have hit enough the enemy, say 9 hits for a total of 27 Guard HP, the HUD will update with a second "Guard bar" to show you got to the next "level".
It's akin to XP and levels, you don't increase your level every time you get XP, it's based on thresholds. If you get a level every 100 XP, you'll need to earn 100 XP before a new level is gained. You never see you're level 12.145, you see you're level 12 until you are level 13. Same principle.
Morale of the story, 3 Guard on hit is like half a penny when you're trying to buy an island. It's worth nothing, especially on Perilous. You'd need 10% Guard on hit to make it remotely useful. You might get some use of it in Routine, but forget about it for Threatening and onward.
