I can go in the Mirror, and make a character that's covergirl. Pretty as can be. they even still look pretty good at the Emporiun.
But leave the Editer, step out in the Digital environment, where your a little ways off when looking at them, and they look like someone added 30 years to there age, and hit um with a really big ugly stick :/ .
That happens to me everytime I start a new game. Seriously - the lighting in the cc and in the mirror is quite special and depending where you are the lighting is much more different. For example at the start of the game the lighting is brutally bright - even a dark skinned character looks really pale in that light. And in most of the cutscenes especially in Skyhold it is quite dark.
So to make things short: I think it depends on the different lighting and I found that characters have something like a "best side" (in german we would say "Schokoladenseite" which I can only try to translate as "chocolate side") as well - from some angles and in some cutscenes they look very pretty and in others, well what you described 
I have the opposite problem. Characters that I try to make look older, appear younger.
