I made all my choices on the keep and loaded them into dragon age inqusition but for some reason some of the choices are wrong for instance carver Is alive even though I chose that Bethany was the one that loves can anyone help me

Loaded world state works only with new games. You can't replace it after creating a character. While creating new character you must be online, so you can import your world state from Keep.

Apart from that I can only think of some problems with Keep, or bug.
