I know I give Bioware endless amounts of flack for Mass Effect 3 and how disappointing it was, but damn if the Krogan campaign doesn't have the best use of choice and consequence I've seen in a game to date. Despite my sour feelings towards how convenient some aspects of it were, like Kalros and stuff, it feels like every outcome has a sense of gratification to it because everything happens according to what decisions you've made and it feels like, for once, you do actually have full influence on how the story folds out.
Even in the worst possible outcomes it still feels rewarding because I made the choice. It feels fair because I know exactly why the things that happen, ended up happening.
Honorable mentions:
Witcher 2's campaign split in Act 1 between Iorveth and Roche.
The Citadel Sidequests - Mass Effect 1
The player-driven approach to Operation Snake Eater - MGS3. (bit of a stretch, but there's so much to discover here)
