Did you make her the new pope? If you told her you didn't need her after the ending, then I think you're out of luck - she goes to prepare for her coronation. If you said you needed her around, she'll stay.

Once any companion leaves for any reason, they are gone for the remainder of that play through. You can load a save prior to their departure if you really need them.

When and how do you do what? Can you be more specific?
As far as I know, she only leaves if you make her divine and tell her she can go be divine, which happens in Skyhold, after the end of the game. If that's what you did, then you'll just have to load a save from before you said she could leave.
