That's literally what I thought of! I couldn't get him on the Avaar Nug though haha.
I had that happen in DA2. I saved right before using the mirror in the emporium, but messed up and exited the mirror and reloaded... And somehow that caused Hawke's head to COMPLETELY disappear when I reloaded the save. Merely reloading the savefile made before using the mirror caused it. It was really freaky, not too different from your own experience with your Inquisitor. Thankfully, going back into the mirror and exiting back out restored Hawke's head.

I had the same issue for a short while after getting into the Jaws of Hakkon. It happened when I tried on some loot I got from a giant. Pretty funny. It sorted itself out in a few minutes.
I need permanent CC option for this.
At least your eyes won't get hurt by ugly hairstyles
I've had this happen, but only during combat and only in the Frostback Basin. It went away after combat, so I didn't think it was a big deal.

Luckily the problem is not that serious, it only happens when I wear a particular helmet and am poisoned. Once the poison goes away the head comes back haha.

That happens only, when you come too close with the camera, e.g. they stand right in front of a wall, right?
I made a standalone module for Dragon Age: Origins, with fixed protagonist, and I could not figure out how to attach her head.
It took me, like, a week to resolve. I honestly thought I was going to have to change the entire story and make her a headless ghost.
I can't imagine this problem is related, engine stuff aside. I had located the head model I made in the wrong place, I think, from memory. But, issues of headlessness intrigue me greatly.
