No legs. 

Background: Your mom was having a bit too much fun in college and did a quite a bit of Red Sand and you were born without legs. Council Disability Act grants you a fantastic biotic wheel chair, so you can roll over your enemies toes in the Deli or something.
 

Mental disorder.

Background: You did a bit too much Red Sand in college, and now you're a tad unstabil upstairs. Your squadmates have to hide during the night, because you'd go on a rampage and smack them in the lips, thinking they are Gremlins.

 

Dwafism.

Background: You're a midget and you have to earn your respect on the battlefield. You're a joke to your enemies, but now is the time to show them how awesome of a shot are you and you can do 22 cartwheels in a row!

 

Obesity.

Background: Sgt. Gardner turned out to be the next Iron Chef and his meatballs are no 1 in the galaxy. You're hooked now you're a lost case to even Dr. Nowzardan's 75th cousin. You can't get out of bed, you have to use your best tactics on the holo-board to direct the battles, while you're hooked up to your oxygen tank with your mouth full of balls.

 

Your condition would get better as you raise the difficulty level.

We already had a handicapped hero in Commander Shepard. Being dead is a handicap...right?

I imagine it would be something like Remember Me's Nilin. A missing limb or possibly pair or entire set of limbs would be replaced by an advanced prosthesis. With robotics and cybernetics as advanced as it is in the Mass Effect universe, there's no real reason why this hero would be bound to a wheelchair.

Got turned into a cyborg. That is the opposite of a handicap.
