I'm posting this to try and find differences in the final mission. Hope you'll join in.
I got the best possible ending after two tries. Went in with all the ship upgrades, all characters loyal, and almost all the other upgrades, including all the character specific upgrades.
First try I ran around doing extra missions after my crew was kidnapped thinking they might contribute, it didn't, and I got to watch the lovely and eccentric Kelly chambers be killed in a rather unlovely way. Dr. Chakwa I sent back alone and she didn't make it.
I also sent Thane through the vents, thinking we'd need infiltration skills, and got him killed when he couldn't close the door.
Otherwise, in both cases where we split up I chose Garrus to lead the fire team, since he proved himself on Omega, where the only reason why he lost his team was through betrayal.
When the seeker swarm became an issue I had Jack on barrier detail with no problems, she was the clear choice being the most powerful biotic there.
Doesn't seem like who you choose to face the final boss matters, but this time I chose Mordin and Jack. No casualties straight through to the end.
Second try, I immediately went to save my crew, ended up with a unnamed colonist melting, doesn't seem to be a way to the colonists though, will have to experiment. Sent Samara to escort the crew back to Normandy since she seemed like the kind of person used to escort missions. No casualties. The only other change was I took Grunt and Mordin to the final boss.
I'm wondering how much can be changed. I'm about to do a run through that I hope will be a bloodbath. Zero loyalty, fewest upgrades. I want to see what and how much changes. With my perfect run, I was a little disappointed about how easy the choices were to make, and I'm thinking that it might have tougher choices and maybe more stuff you have to go through if you don't have everything ready.

Had all upgrades for squad members and Normandy sent Tali to the vents took Legion and Grunt with me and Miranda Leader of 2nd team first time then Samara hold of the bugs and Thane and Grunt. Sent Garrus back to ship.. and Miranda and Mordin for final Boss

Vents: Jacob (he volunteered and I figured since it said it would be dangerous that he'd have to fight)

Got to survivors, sent them back with Jack

Against the terminator: Miranda and Thane
End result: Jacob got a rocket to the face and Grunt didn't make it (I didn't have the his specific prototype upgrades... but come on, he is the PERFECT Krogan...). Jack got everyone back alright (Jack didn't have her prototype upgrades either).
While Grunt and Jacob did die I kinda liked the end result (I liked Wrex better than Grunt and Jacob felt like the new Kaiden) if everyone lived it woulda felt kinda lame (ooh suicide mission were no one dies, really?!)

No casulties with the barrier team in fact Jack concentrated the barrier into raw power and owned all those collectors chasing us. Grunt however, did not survive
Tali made it though with the crew though. 
I had both Legion and Mordin with me during the final battles. Both survived.

Never had a squadmate die in any playthrough. I did it right the first playthrough as well with no guides. Most of it is common sense. Have all team mates loyal.
For the vent I use Legion
For Fire Team leader I use Garrus
For Escort I chose Mordin
For Seeker Swarms I used Jack
My team always consisted of Miranda and Samara.

Jacob in the vent. Numbnuts volunteered, so he got the job. And a bullet in the face closing the door.
Zaeed as 2nd team leader the first time, gutshot, dead.
Can't remember who I picked as 2nd team leader after that. Garrus, maybe. To mix things up, I did one thing right.
Nobody escorted what was left of the crew, so they died.
Miranda for the ME field. Grunt got taken by the Seekers.
Can't remember who I had with me in the final battle, again, but Mordin died holding the line.
3rd time:
Pretty similar to the first. Definitely ALL of the character-specific upgrades done, all of the Normandy upgrades, everyone loyal except Zaeed.

Miranda and Grunt with me against the Reaper larva.

I was trying to get everyone out alive in the last one again, so I was pretty worried about Zaeed. Almost immediately after starting the last part and taking Grunt with me, I realized that choice might end up killing Zaeed; my understanding of Hold the Line is that the remaining characters need to have a high enough overall defense skill for everyone to survive, so taking the toughest character away from the group wasn't the smartest choice. But it turned out okay in the end.
Currently on my fourth playthrough. Don't know what I'll do yet. I'll definitely save the crew, though, because I want to boink Kelly this time.

Crap. I had a nice explanation written out and it got eaten by the forum monster.
Anyway, your mistake was having Grunt escort the crew back and/or taking Garrus with you to the final fight. Every squadmate left to hold the line contributes to an overall defense score. Grunt, Garrus, and Zaeed are your strongest defenders. Mordin and Tali are some of your weakest. By taking both Grunt and Garrus out of the equation, your defense score wasn't high enough to ensure your weakest defender will live.
Most would recommend sending Mordin to escort the crew if you're not taking him with you to the final fight because he's the one with the weakest defense, and thus, the one most likely to die.
You probably don't need Grunt and Garrus to hold the line, but you'll need at least one of them.

I've beat it on insanity with only one character dying... FREAKING TALI! You ant-eating ****! I could've gotten two achievments in one fell swoop! But I've completed many playthroughs and I only have had everyone live once. <_<
Oh and I completed the suicide mission with everyone alive without activating Legion. It was much less agravating.

To update what you see in that post, a loyal Kasumi can also serve as your tech.
You actually don't need everyone's loyalty for everyone to survive, it's just the best way to ensure they do. If anyone was unloyal, the only way to keep them alive is to simply not use them for any specific task during the mission. It's not a guarantee, but it is doable.
Of course, there's an exception to the above. Miranda can be unloyal and still survive as the leader of the second fire team. She's the only one.
Another thing you can change and still have a positive outcome is the squadmates you leave to hold the line. Grunt, Garrus and Zaeed are your strongest defenders, but that doesn't mean all three must hold the line so that everyone lives. You can be missing at least one.
Also, I'm not sure, so I don't know why I keep mentioning it, but I think Jack's biotics need to be upgraded in order for her to successfully hold the barrier. Maybe I should actually test that theory instead of mentioning it all the time...

Unfortunately Thane died on my first playthrough (vents) as well as the crew because I thought the person that tried to escort them would die. My other playthroughs saw no casualties.

Took some casualties early on; thane was killed by a stray bullet and only half my crew was still alive inside the collector base. I arrived Just in time to watch kelly die screaming in pain as she was melted into grey sludge. After that I Decided my men could use a pick me up and gave a stirring speech. sent zaeed to escort the crew and had miranda lead team 2 while Legion Tali samara and I headed to the center of the base. no further casualties and tearfully I decided to save the base because as much as I hated TIM and as much as I wanted to blow it up I knew he was right and having reaper tech would save more lives in the long run; I hated him for it. I plan on finding a way to shut down cerberus in ME3 to prevent misuse of the base though.

I had all of the upgrades on the ship. (Except for the Medical Bay which I did not need anymore).
Everyone was loyal except Zaeed.
I sent Legion into the vents.
I had come to value Jacob's imput to the building of the mission so I wanted to place the rest of the team under his command. I took Thane and Garrus with me. We reached the Colonists no problem. I had taken my sweet time going ont e suicide mission, but that was OK because there as really nothing left to do so crew was still alive.
I chose Jack to be the field maintainer and took Tali and Grunt with me. I told Samara to escort the survivors back, hoping that she'd be able to rejoin me for the final part but I guess not. I assigned the rest of the crew under Garrus' command because he was the oldest and most trusted friend I had on the team.
We got through the seeker swarms OK, and Garrus brought the rest of the team back fine. I decided to take Mordin and Jack with me to the final battle. The rest of them held the line. Jack and Mordin survived the rubble and everyone made it home safe. To my surprise actually, didn't expect Zaeed to survive but he did.
I had actually spend quite a long time desperately trying to win enough paragon points to regain Miranda's loyalty for most of the endgame. I couldn't quite make it, then I realized all i really needed to do was retain as a Champion. Turns out I probably didn't even need it.
Everyone was loyal except Zaeed.
I sent Legion into the vents.
I had come to value Jacob's imput to the building of the mission so I wanted to place the rest of the team under his command. I took Thane and Garrus with me. We reached the Colonists no problem. I had taken my sweet time going ont e suicide mission, but that was OK because there as really nothing left to do so crew was still alive.
I chose Jack to be the field maintainer and took Tali and Grunt with me. I told Samara to escort the survivors back, hoping that she'd be able to rejoin me for the final part but I guess not. I assigned the rest of the crew under Garrus' command because he was the oldest and most trusted friend I had on the team.
We got through the seeker swarms OK, and Garrus brought the rest of the team back fine. I decided to take Mordin and Jack with me to the final battle. The rest of them held the line. Jack and Mordin survived the rubble and everyone made it home safe. To my surprise actually, didn't expect Zaeed to survive but he did.
I had actually spend quite a long time desperately trying to win enough paragon points to regain Miranda's loyalty for most of the endgame. I couldn't quite make it, then I realized all i really needed to do was retain as a Champion. Turns out I probably didn't even need it.
