She looks closer to mid to late 60s. Cassandra is 35-38 in DA:I? So 35 + 15 = 50. I think she looks older than fifty in that pic. It's a very good representation of an older Cassandra.

Why would she feel insecure? She very much revered her brother, naming her son after him would show how much she loves both.

Cassandra could very well project her brother's life onto her son, hoping that Anthony could somehow live on vicariously through the boy. When in actuality her son (if things go well) will outlive Anthony's meager existence, and will need to grow to be his own man. An overbearing mother is bad, one who wants you to be just like her revered dead brother is hellishly worse. Naming her son something different than Anthony, would be a good first step in keeping that psychological situation from happening. Even if the name is surreptitious, e.g. Mark-Antony, Cassandra is acknowledging that her son is a different person, while at the same time keeping her brother's name alive.
