A SmashBro landed in a GIP I was in recently.
He was working the corner wallz of the Vancouver upper accounting office -- whippin' 'em but good.
My GE plopped the healing turret next to the SmashBro as I was firing and OL-ing through the open windows. I heard the shield charge go off...and then there was a pause in the Smash action.
Didn't last that long, but it got me concerned for a sec. Was this likely just a momentary coincidence? Making sure...
I think the game glitches out in various ways when you get hit and healed at the same time or hit while dodging.
If you are hit while dodging as a Fury/Slayer/Cabal or hit while SS'ing as a Shadow you can become invisible. (fixed by dodging in all directions)
If you are healed while getting shield/healthgated at the same time, you can get stuck in permanent "red death screen" (until you lose shields again) or your powers/guns can stop working (unless you melee)

This could very well be the reason. It's hard to scrach while you're in armor. He probably had to release some omni-nano bugs to scrach him which obviously consumes a cooldown.

Cooldown...exactly. You see that's where me & mah ointments come in. It's just 'ointz' if yr cool & stuff...

To be honest, I'm kind of amazed that I have never experienced this one. That having been said, I never really play the Shadow off-host. It could be that the issue has a correlation with lag, or it could could be that I rarely host anymore, and thus rarely play the Shadow. I have seen invisible teammates before, but there are a few potential causes for that one.
Edit: Yes, I re-read that last sentence, and decided to keep it anyway. Also, it was intentional.

This one is the bane of my existence as a Shadow (or Volus.) Drives me nuts.

This one I've only ever had from specifically being shield-boosted. I don't recall ever having it happen from a Geth Turret or a Shadow Strike.
Ohgawd... if that was a side effect of Shadow Strike... I'd have to Heavy Melee right away! 
Which I already do a lot of the time, anyway.

I don't think it's as simple as getting healed during reload, I've used GE extensively with slow-reload weapons like Claymore, Wraith and Saber, and gotten my weapons stuck literally just 2-3 times.
It could be that your own turret can't jam your weapons/powers.
I've definitely had my powers locked up by my own turret while puggernauting. Melee still works to fix it.

Same, and my experience reflects that. I can't disprove that it can happen from other shield boosters, but I've used geth shield turret and pylon a LOT and have never had them lock out the weapons/powers on either myself nor other players in the match. Getting Volus shield boosted while you're reloading is the only thing that I have seen evidence of.
