Same as the last one, but in the area where you fight the gunship in the Eclipse base during Samara's recruitment mission (happened to me a couple times, but the saves I had where that happened are all gone because my PS3 got stuck in a File Restore Loop and the only way to fix it deleted all the data on my PS3).

Okay, some of these will be really random

1. Dying of old age.
2. Exploring a new toxic environment and you forgot to put your helmet on.
3. Doing a space walk without your spacesuit.
4. The Mako gets stuck in a planet crater and you die of starvation.
5. Exploring a water planet when you can't swim. (You don't have the underwater mech this time)
6. All the galaxies reporters join the "anti-Shepard" union and you have to answer their questions otherwise you receive a death punch.
7. Andromeda species unite against the protagonist with pitchforks. Died through getting skewered.
8. Your on a life-support machine and a Geth/Quarian accidently uses their hack ability nearby.
9. Visiting an Ardat Yakshi brothel.
10. You pass a Shifty Looking Cow with no money. It takes your life instead.
11. Walks out to a stormy environment and then gets struck by lightning.
12. Biotic skydiving. Instead of opening a parachute, you use biotics to stop the fall. You forgot to use your biotics.
13. Elcor love interest. The Elcor rolls over in bed and squashes/suffocates you.
14. Trying to pickpocket a Volus. The Volus caught you and turns round and says "Big *deep breath* Mistake". Then shoots you with a shotgun.
15. Didn't throw a grenade far enough. Death through explosion.
16. Getting swallowed by a Thresher Maw.
17. Playing Kepesh-Yakshi on the lethal shock setting. You lost. 
18. You forgot to look both ways and got killed by a Skycar.
19. You visit a planet with stronger radiation than Haestrom. It doesn't just affect your barriers but causes instant incineration. You didn't stay in the shade.
20. You lost a bare knuckle fight with an Elcor.
