So... I was battling Northern Hunter the other day. Suddenly without any warning Sera turned into an Eldrich Abomination with the size of Harbinger. It is funny because of the atmospheric fog which gives a sense of scale.

That's it, Sera became evil Andruil with armor made of the void itself again! Either that or the reason she hated magic was that she was actually a self hating mage, a very powerful mage that become an abomination 10x stronger than Nightmare when she got possessed. She even looks like a spider! Or maybe she's still on your side? If so, the game will be easy mode now. On nightmare, versus the level 23 high dragon, she could just step on it and kill it in one hit.

The only one I've ever had the fortune of witnessing is enemies who balloon in size when they're killed (I've seen it on a dragonling and one of the smaller critters—the model inflates to several times its normal size after they die).

But nothing this glorious.

So this is why she prefers Qunari girls, it reminds her of her other form being so tall

So, I'm thinking this must have been caused by her falling out of/being teleported out of the map. You have the actual region map that you can run around on and then you have an inaccessible backdrop (mountains/sea/sky) which is called the skybox. However, skyboxes are actually much, much smaller maps that exist in the same game space as the explorable map - they are just physically located a long way underneath the explorable region.

 

These skyboxes are then magnified and projected as the background you see when you are running around the explorable map. Sera hasn't actually grown, she's standing in the relatively tiny skybox which is then being magnified and projected as the background to the physically explorable map - but she's getting magnified along with the skybox which is making her seem huge.

 

This might be the wrong answer, but it is the most likely answer and it's a bug that exists in other games eg., Counter Strike. It's called skywalking in that game and is a bannable offence if done intentionally, so you might want to give Sera the "You don't fit the Inquisition" talk, OP. You need to set an example to show the other companions that this type of behaviour will not be tolerated.
