I think there's different flags for Kelly's friendship and romance. I didn't get Kelly's picture although I did get the fishes and the dialogue was different I think, she said it herself that she thinks Shepard as her close friend.
IF I recall correctly... the trigger for romance/friendship is how you respond to her "if I fell, would you catch me?" dialogue. If you say you'd catch her, you're on the friendship path. If you say that you'd embrace her, you're on the romance path, though I think a later dialogue allows you to back out of it down to friendship. (She checks that she isn't coming on too strongly.)
Alternatively, I believe the "would you catch me" dialogue can be selected more than once, and, because of how the save flags work, if you ever say that you'd catch her rather than embrace her, the game locks you into the friendship path.
Friendship path still has the two of you having dinner in Shepard's cabin, and Kelly still offers to feed your fish. Given that Kelly is a trained psychologist and counsellor, I imagine that the dinner is her giving Shepard a chance to unload after everything that the Commander has been through.
