When your search option suggests that you should investigate the area, is it always something that will show up when you stumble across it, or could it also be someone that you should speak to, or a fast travel suggestion?  

 I haven't been able to find any of these things. Will you only find them when using the V option (PC) or can you just wander across it?

It is an item that you can't find otherwise.

Its most likely a quest item, like blackwalls warden items. Or one of the bottles of thedas. And yes you can only find it using the V key.
