Approval system that gates quests sucks!

These sort of quests shouldn't be gated by approval, it's simply stupid. Bioware should re-evaluate.

You can earn that approval back pretty easily by just locating a few Grey Warden relics.

I sabotaged Viv's potion and had her lover suffer a horrible death. She gave me **** about and ten minutes later I start up a convo and she calls me dear.

Well, I mean it kinda makes sense. He doesn't respect your Inquisitor enough and your actions haven't pushed him to become a better man. You can't please everyone sometimes.
He should respect me. This arbitrary system is pointless since he's been in my Inquisition for quite a while and bleed for it and me. I think I have his respect but because of two choices and a stupid approval system in check I'm not able to see his character progress.

While I do shake my fist at some rep gating (like a few rather absurd Paragon/Renegade options pretty early in some of the ME games) not being able to do everything when you actively do things to tick a character off isn't bad game design.

I may just play a game in which I don't recruit Blackwell, Iron Bull, Vivienne and Sera. They all get on my nerves and I actually dread having to talk with them. How many experience points would I lose?

Don't know xp wise, but you lose the wartable operations if that matters.

You get about 1025 XP for each personal quest, plus there are the collection quests for Blackwall and Vivienne. I would say about 5000-6000 XP, plus the aforementioned War Table operations. Of note; you need to recruit Sera to get the Jar of Bees.

I did this with Sera and Cole in my new playthrough and enjoy it a lot more, definitely recommend skipping characters you don't like.

Inquisition is not realistic at all. It is the same number **** from previous game it is just harder to access numbers since they are not available for you in game anymore, but it is still the same. Not that it matter I always get max approval with everyone I care in all Bioware games, most of time with people I don't care too, like Carver. Bioware games are disgusting when you play them sponteaneously, I just go to the wikia, approval pages choose quests to take or leave companions based on approval/disapproval and bingo, always max with everybody.
But I can imagine the pain of people who just play the game. Not being able to choose the outcome of WEWH is another sad thing. IMO RPGs shouldn't make it hard to make the story go the way you want, neither you should need a guide nor multiple playthoughs to finally make the story the way you want. But the developers are dumb and they think it is funny making people play their boring game so... well, read the guides, read spoilers, read everything, it is better to spoil the "surprises" than having to finish the game 10 times to know how to make something you didn't even know it was possible.
Well, unless you are one of those people (who I will never understand) that have joy in discovering new things, if that's the case, have fun with your masochism of having unwanted outcomes in various quests. I'm not into effort, I just love things handed down to me on a silver platter and wikia (among other sites) does that just fine.

That being said, some of the "Approve" and "Disapprove" instances are just odd. There were several times I found the approval system hypocritical or contradictory. Some of the companion's approval or disapproval made no sense and was very chaotic.
A little revamping may be in order, but I kind of understand personal quests. I wouldn't go to my supervisor for help with a personal issue simply because I respected him/her, but I would go to my supervisor for a personal issue were they a trusted friend.
