I feel like this entire game is filled with design choices that are specifically put in place to infuriate players with things they can't control but why does loading an auto save create another auto save? That serves no purpose at all except to make players lose as much progress as possible.

Are you sure the game is actually saving? (Though it shouldn't make too much difference, since the interval save is something like every minute, and the other save is for loading an area or resting.)
Really the only scripted save is the milestone save. The others typically hover in various stages of uselessness. You should be saving manually in Inquisition.

Yeah it actually did save. I accidentally forgot to save that guy in the western approach because apparently he can just die?
I know I should. But as always just because something SHOULD happen doesn't mean it does. Figures the one time I forget to routinely quick save is when I need it.
