I trying to finish off landmarks and regions in the Western Approach. Best I can figure, I'm only missing the Gates of Andoral. Found and opened the Gates of Toth, but I must be blind because I can't find Andoral.

I think you need to do the quest where you find the source of the Darkspawn , there's a mini dungeon and  you exit near the gate?

You know how after you leave Coracavus, you go left in order to find Servis and the Gates of Toth? Go right instead.

 

Also, have you gotten the Thing in the Cave landmark? That one is easy to miss.
