"Ho there" is... Something. I think it's a form of address, Cailan uses it to greet Duncan in Ostagar.
But I guess that isn't quite what you're asking. To be honest, I don't recall any particular form of address. They just used normal ones, like sir (or it might have been ser) and madam. Same for Orlais, I suspect, since I don't recall any particular form of address being used in DAI either.

That's a greeting.

"Mi Lord" for male nobles seems pretty common (the Revered Mother in the City Elf Origin addresses Vaughan as this), and "my lady" seems pretty common for female nobles, and "ser" or "good ser" for knights.
Peasants...? I imagine it's something like this.

King/Queen: Your Majesty
Prince/Princess: Your Highness
Teyrn/Teyrna: Your Lordship/Ladyship
Arl/Arlessa: My Lord/Lady
Bann: My Lord/Lady
Lord/Lady: My Lord/Lady
Knight: Ser
From WOT1. Doesn't say anything about commoners. I don't think there is a proper form of adress for Fereldan commoners, I certainly don't remember anything like that from DA:O.
