Abilities can be reset with the Tactician's Renewal amulet which is sold at the Blacksmith's forge.

Look for the banner-looking thing next to the table where you tint armors in the crypt in Skyhold. It sells those amulets. Once you equip it to the character you want to reset, it will "destroy" and you will be able to assign all your skills point de novo.
(I did it 3 times for all of my mages because I'm bad at playing them
