I'm currently trying to romance Sera as a level 13 human mage. I flirted with her from the start and avoided making decisions that she would disapprove of, but I have been stuck at the stage where when if I ask her where the relationship is going, she just gives me the "you're nice, but lets travel around more and we'll see" for quite a while.

So I'm starting to worry if there's some glitch effecting her approval rating. I first noticed during the quest "Under Her Skin" that when I agreed to mercifully kill Erasthenes Sera, who was present in my party, should have slightly approved, but the message on the side of the screen only noted that Cole approved. I reloaded a save and tried other options, which to which it noted that Sera disapproved. The same problem occurred in the next place I went to, the Emerald Graves, where when I flirted with Harding, to which Sera usually slightly approves, no message noting her slight approval appeared.

Did anyone else encounter a similar issue? Is it indeed a glitch and if so, what must I do to successfully romance Sera? Cheers.

There is a cap to all companion approval. If you stop getting "known" approval, but can get disapproval with a different answer, than that is usually a clear sign you are at maxed approval with said companion.

As for Sera's romance though, unfortunately I am not familiar with it yet. Hopefully someone can give you more/better insight.

How far in the story have you progressed? You have to complete certain story arcs to advance her romance. Her relationship is worth it though, good luck!

Did you complete Verchiel March? It's her personal quest, once you complete it and get back to Skyhold, ask her if her "group is still active." After that conversation path, tell her again that you are "interested" in her, and the romance should initiate.

Make sure you've exhausted all the conversation options open to you except kicking her out. You need to explore them all, even the one that is paraphrased as something like "you're unusual for an elf" in order for the romance to progress.
