I'm seeing Here lies the Abyss and What pride had wrought showing up even though I've yet to get to them.

 

This is with the keep set to not show spoilers.

If you have previously unlocked the default options for any choices then they will show up before you reach them in subsequent because there is no option for "have not done yet" and option unlocks are account not playthroughs specific.

If that were true I should be seeing all the tiles, not hidden sections.

 

For more info this is a worldstate imported from the game, that has yet to do them.

If you are still seeing some sections hidden, then what is likely happening is that the choices you've unlocked for that tile are simply not selected; if you've unlocked tiles in that group before, you should be able to see and manually select them (without impacting your DA:I game) if you like.

 

Randiel Marius is correct - the Keep only treats tiles as spoiled once, until you've seen it in at least 1 playthrough.  From then on, that tile will always be shown, regardless of the spoiler toggle setting - that setting only affects plots you have not yet encountered or unlocked in any playthrough.
