By the end of my playthrough I had to choose between Cassandra and Leliana, assuming they were the only candidates. I ended up choosing Cass because I liked her plans and idealism. So to my surprise (and I LITERALLY ONLY JUST found this out), Vivienne is a candidate as well. No one in game ever mentioned this to me - not even Vivienne herself, despite establishing a friendship with her and giving her everything she asked for.

 

Did I do something wrong? Why did she never bring this up to me? Not that I would have chosen her anyway because she is a bit of a tyrannical b*tch.

She doesn't bring it up herself, but at some point at Skyhold you will get a cutscene with her where she starts the conversation with "Inquisitor, i know you must be very busy, but..." or something along those lines. If you have high approval with her, when she's talking about Cassandra being the best candidate, you can tell her that she'd be a good candidate. You'll get a wartable mission to support Vivienne after this cutscene if you chose to tell her she'd be a good candidate, or if you agree'd on Cassandra being a good candidate.

That's a problem I had in my first game. Viv got it instead of Leliana, who I put in the good word for and talked to her about it. I never once talked to Viv about it nor was my approval that I high because I never really did anything for her.

Vivienne is subtly making a bid for your support when she discusses the Divine election - look at the qualities she says a Divine should have, and compare them with how she'd describe herself. Perhaps too subtly for her own good, since people do apparently miss this.

And I do think the game is probably rather too subtle in general about the prospect of her candidacy. It doesn't really make sense that no one would have got wind of it, considering what a radical step it is.

I believe I read somewhere that if 2 candidates are equal in points, Viv is automatically given the win. (?)

Vivienne has the priority over the other candidates in the case of a tie. In the case of tie between Leliana and Cassandra, Leliana has priority.
