Internet in my area is dodgy at best, and I frequently drop out of online mode without warning. Well, as luck had it, I was just in the process of completing In Hushed Whispers with Travel Light on when my internet cut out. I was in a cutscene at the time and had no opportunity to save, reconnect and reload. So now, I've got the quest completed and no achievement. Bravo.
Online mode required for DLC achievements?

I thought the game recorded achievements and uploaded them upon next Internet connection. That's how it worked for me. I got a ton of achievements when I connected after I finished the main game. Maybe the new achievements are different?

Achievements definitely work offline for X1 and PS4, and I believe PC as well.
Also you need to have the trial turned on for the whole mission.

I'm on PC.
But maybe you've identified the issue. I turned it on before committing to the War Table operation, but it wasn't turned on in Val Royeaux when the quest was assigned, nor was it on in Redcliffe when speaking with Alexis or closing the Fade rift. I loaded a save that was standing right in front of the council door, ready to take the 15 point mission.
