You can do every mission in the game and still get Shepard and all 11 squadmates killed (not sold or inactivated = killed).

Saving everyone is pretty easy assuming everyone's all loyalty'd up.



The guide on the wiki walks you through it fairly clearly. Basically:



Tech Spec: Tali or Legion

Fire Team Leader(s): Miranda, Garrus or Jacob

Biotic: Samara or Jack



Hold The Line: Your toughest characters. Basically, on hold the line, Jack, Mordin and Tali have the highest chances of dying (if anyone's gonna), so send one of 'em back with the crew and take the other two with you to the boss.



Easy.

I've finished the game 3 times, never had anyone die.

I pretty much do what's listed here, though I've always used Mordin as the escort. Sometimes use just Garrus or Miranda as leader both times. I like having Thane and Samara for the last battle.

I've seen some comments that everyone has to be "loyal" or else someone dies. In my 2nd playthrough, Zaeed and Miranda weren't loyal, and no one died.

No, you don't need everyone to be loyal. You can still juggle it with unloyal people so everyone lives. It's actually a little more tricky figuring out exactly how to kill off precisely the squaddies you want to die.

Everybody but Shepard, Tali and Zaeed died.



Jack, Garrus and Thane died on the ship.

Jacob died in the vents.

Samara died escorting the crew.

Mordin was carried away by bees.

Grunt died leading the second fire team.

Miranda died when she took a nasty fall, leaving Shepard and Tali to make a mad dash to the Normandy while Zaeed held the line all on his own.



Only way it could have gone better is if there was any way to keep Garrus alive on the ship without upgrading.

I've finished the game with 5 playthroughs and redid some of the collector base to get the cut scenes that I wanted. So far with a fair number of playthroughs between Vet, HC, Insanity I haven't had any deaths.



Then again I just did what the game "wanted me to do" so it wasn't rocket science.

Legion was my main casualty.  Died every time until I loaded an earlier save and used the Paragon option in his argument with Tali.  Mordin died once when I brought Miranda with me to create the biotic shield.  And Tali died when I chose Zaeed instead of Garrus to lead the second squad.

Lost Tali in vent and Samara during the swarm first time.



Always seem to loose the Vent person, even though I have max loyalty for all and almost all upgrades.

u need miranda or garus (loyal ofc) as fire-team leader

First and canon shep: Legion died as tech specialist, Mordin was my 1st fireteam leader.  Everyone else survived except legion.

Second time: Never got Samara and Miranda died at hold the line.  Everyone else survived.  Which is somewhat annoying since I was trying to kill at least 2 people this time around.

my first playthrough everyone survived.
second playthrough i never woke grunt, and six people died. Thane and Jack in the space battle, jacob as tech specialist, zaeed as distraction team leader, garrus got carried off by seekers, and miranda died holding the line.
all those deaths except garrus were semi-intentional.

*sighhhhhhhhhhhh* I am predicting massive character death on my first suicide mission. My paragon meter hates me, Zaeed's already a lost cause, and I can count on losing Jack and Legion's loyalty and getting Tali exiled from the Migrant Fleet. Hate the morality micromanaging. I think this is because I didn't level up my character class soon enough.
