Hawke is only mentioned in that epilogue slide, but isn't shown.

I was really happy to see my Hawke living happily in Kirkwall with her BFF's. Also more than happy to headcannon that she and Izzy live together and she uses Kirkwall's ports as her base of operations. Everyone's happy
Also enjoyed that Varric continually ignores Sebastian's letters

Yep, this does change based on who you left in the Fade. Mine had Stroud instead of Hawke. You can read all the different versions here. It's kind of depressing really, but a neat touch.

That really is depressing but I love it. Looks like it was inspired by that piece of fanart about aged Varric remebering his friend Hawke who stayed behind in the Fade. 

 

I really hate leaving anyone in the Fade, but luckily Hawke always gets out with my Inquisitors.
