Tbh I liked how most of squadmembers in ME3 called me Shepard much better than the Inquisitor....it felt kind of impersonal. I want to have a surname and that surname to be used by my comrades in arms.
If we are addressing a superior or a relatively new person then the title "Commander" or "Captain" (or what is going to be) will do.
ME got it right to me. I also loved how I could tell to Vega to quit calling me Loco. It's either Shepard or Commander.

Would prefer a fixed last name like Hawke or Shepard, but would settle for Captain if I can have that N7 Paladin Shield from ME3 Multiplayer

I wondered if it might be Captain or maybe something a bit frontiery like Ranger or Marshall.
That would probably be best, not obviously higher than Shepard's rank like Admiral or Captain would be, and it gives it's own unique twist, probably the best option unless they just go with Pathfinder.

Too early to say at this point for me. Since we are leading an expedition to explore and colonize new worlds, meet new races and forge alliances, being Captain at the start doesn't seem far off. Anything above that... no. I would prefer them to keep the ranks possibly at the lower-mid end of the spectrum to allow our hero to prove themselves which would open more interesting story elements imo. The leak detailed that our hero is supposed to be combat trained but untested, which sounds like a prodigious but inexperienced specialist. We will probably have a surname which we will be called by, like the ME trilogy.

How can someone that BioWare has stated "becoming a hero" already reach and out rank Shepard who we start the game off already a hero and immediately gets promoted to lieutenant commander?
Doesn't make sense. I don't think we'll even start the game off as an officer, let alone an executive officer.

Whos to say. You don't need to be a hero per say to become a captain. I also noted "lower-mid end" of the ranking spectrum to allow for more character development and alleviate the issue you are worried about. The protagonist will definitely be a person of authority, whether it will be rank, charisma, or both.
