A friend and I were comparing notes and we both noticed that when we go out on a mission with our companions we hardly ever get party banter anymore. We get comments from our companion about a given quest but nothing else. Seems like this started happening after the latest patch (10). Has anyone else noticed this?

This has been an issue for most people since launch, actually. I did notice that it seemed to play properly in the JoH zone though, for whatever reason.

For me it was better before the last patch, started new game and now companions don't even talk in Hinterlands, and none of the usual workarounds work. Have to use CE banter trigger now.
I hope this overcomplicated, bug prone banter system will not be present in the next DA game.

I thought the new updates had improved that situation. I've started a new game recently and am noticing it is a lot better than it was at least.
Basically after every patch was released, you had some people saying it was fixed, some people saying they got almost no banter after having a lot, some people saying it did nothing and they still had a lot, and some people saying it did nothing and they still had none. So I would say none of the patches actually did anything. But who knows? They don't release full patch notes.

I used cheat engine for that, is awkward 10-20 mins walking in any place and they only have one banter!
With the cheat engine is more realistic, but if you are going to romance a companion is better you wait.

The party banter in this game is really weird. I always felt that it happened way to rarely, but in my recent two playthroughs I noticed that there where differences based on the area I was in. I always got lots of banter in the Hissing Wastes, the Forbidden Oasis and Emprise du Lion, but close to none in the Western Approach and the Hinterlands. The banter in other regions was medium or rare.
I've never used the cheat engine before, but I think I will in my next playthrough. There's just way too few banter, I didn't even get all the conversations with my go-to Team, let alone all the other companions that I'd like to get to know at least little bit while walking around.

I'm not sure if I'm getting the same amount of banter as before. I'm at the end of the game after all, and playing Descent right now. But I was still getting banter while playing Jaws of Hakkon. I haven't had any among party companions while playing Descent.

I ended up using cheat engine to trigger the banter all the time. (well every 2 mins) Got fed up of pressing the key
Plus I wanted Iron bull and Dorian to start their romance sharpish

Checked in with another friend and she traveled Crestwood, Storm Coast and Western Approach with Cole, Cassandra and Dorian for three hours. During that time only Cole made one of his strange remarks about demons not liking water.

My banter was never fixed. I hope in the next titles they return back to non-timed random banter, as the silence is quite oppressing with banter problem and intentionally limited music.

The only thing I noticed since the last patch is that in the Hinterlands once you get 4 power Solas keeps on whining that you should go talk to the chantry.
Never had that happen before in all of my playthroughs.
For the rest the banter pretty much seems the same.
Also, for people that want to listen to banter, once you finish the main quest in Descent there is pretty regular banter in the Deep Roads. Don't even need to move around for it to trigger. I was abusing the war table cheat for a while and the banter just kept on coming. Might be worth it to bring some party members that you don't usually use.

The thing is though: I have heard the banter - by standing in Hinterlands still for hours. I rather just hear it organically while actually playing.

The problem for me is that most of the thing I'm interested in doing in the game make you ineligible for banter.

I recently started a playthrough (being done with TW3) and I've played about 5 hours and i have still not heard a single conversation.

That's happened to me since launch, I averaged about 10 banters in an entire playthrough no matter how much I switched my party members or that I didn't use a mount. (I don't count the "we should look around" comments)

You're not alone.
I remember having normal banter at first. At some point after In Hushed Whispers either one of the early patches or some in-game event completely screwed up something and all banter with the exception of event specific reactions like "Apostates!", "Templars!", "Check it out, it's mother Giselle!" etc. ceased. Uninstalling and reinstalling at three different points in the game didn't do anything. Reloading a very early save didn't do anything. Changing the party configuration didn't do anything. Avoiding fast travel didn't do anything. Traveling exclusively from the war table didn't do anything.
I was hoping a bigger DLC would fix the issue so I could finally do a second playthrough. I bought and finished Jaws and Trespasser (great DLCs even with the banter bug) and nothing. I started a post-DLC test character and after about five hours in the Hinterlands the only thing I get is Varric saying "Does anybody else smell that? Just me?" every half hour or so. Now, if the music behaved more organically, like in TES for example, it wouldn't be as frustrating, but as it is I feel like I'm traveling in absolute silence.
I was overjoyed that I could manually trigger some banter in Trespasser and I wish that were an option for the rest of the game. It definitely beats watching on Youtube what I should've experienced in the game.

I heard a rumor that there was party banter in Trespasser, do people mean when you click those little glowy things to listen to a companion comment, or when they comment on things you come across such as the elven murals you pass through, or am I missing out again?

It's a new problem for me. It used to take around half an hour of walking around for it to trigger, but now it takes forever, even in JoH where it seemed to happen more constantly before. It's really frustrating, because it's the first time I'm playing JoH after having finished the main content and I was hoping to hear what they have to say now, in a different context. Dorian, Viv and Cass only talked after two hours and it was something quite random that had been triggered before, in the same playthrough.
