I had a similar problem and it seemed to be because I had two of the same versions of the item I was trying to add. (As in I had two leggings of +7 cunning or something like that and kept getting the error. I then broke down one of them so that I had only one of that type in inventory and it them worked.)

Luke said its a known problem and that they would fix it next server push but that was a week ago and we've had qunari added since then with no fix so Im not sure when this will be fixed.
