I just finished a playthrough where I was trying to rush through everything intentionally just to see what sort of suicidal ending I could achieve.  Yet somehow my Shepard survived. Sure I still got most of my crew killed, but I wanted to see what it would actually take to get Shepard killed off. 

So I guess the question is, what did I do right? Has anyone been able to get everyone, including yourself, killed off in the suicide mission? And if so what does it take?

I would have figured the dying bit wouldn't have been quite so difficult...

If your normandy isn't sufficiently upgraded, it might not be able to escape the ship as it explodes?

Maybe if you do zero upgrades? The absolute minimum walkthrough.
