More specifically, at which point in the story and at what level? 

I'm doing a third playthrough (to drown my sorrows from not being able to play Trespasser), and decided to romance Sera again. I remember having some trouble gaining enough approval the first time, but it worked out in the end. This inquisitor's a sarcastic, "woman of the people," so I gained a lot of approval quickly. In fact, I think Sera's approval is maxed out because I no longer gain approval from her for decisions she approved of the first time. The, "I'm interested in you," option is still available, but Sera still gives her speech about needing more time and travelling. 

 

I've played pranks with her, eaten cookies on the roof, done all of her war table quests, gone through the Verchiel March. I'm at level 12, just finished Adamant. I haven't been to Halamshiral yet. 

 

So at what point did everyone successfully start a romance with Sera? I'm really hoping that this isn't a bug.

The most common reason for the romance stalling is that you need to exhaust all possible Skyhold dialogue options short of kicking her out, even the one about her not being like other elves. If you don't do that then it won't trigger.

Did you do her quest after the verchiel march? The one with the noble?
