and can I change the worldstate during gamepay, with cheats or mods  ?

No, I don't believe you can do that.

I believe you can use a Save Editor to change your world state by editing a save and that the Save Editor comes with a note detailing which lines are plot flags and such.

You mean edit the game to have Alistair show up instead? When all you need to do is set it in the keep

The thing is I am half through the game and just now realised I want different choices than Default I cant use the keep at this point can I ?

There is also a place where you can choose Alistairs fate in the keep - choose "stayed with the wardens"

The Keep is preferable but it cannot influence a game that has already been begun hence the Save Editor which I believe can. It's a bit more difficult than the Keep.

It would be easier and wouldnt break the game to do it via the keep

You can't have the HOF and Alistair both grey wardens? Can't you have a romanced Alistair be Grey Warden with HOF? I am confused.
