What determines Carver and Bethany's fate?

It's dependent on the player's choices in the Deep Roads quest at the end of Act 1.
Leave them behind - circle mage/templar
Take them, but don't take Anders - death
Take them, and take Anders - they'll contract the taint, and you'll get the option of death or joining the Wardens

If you play as a mage, Bethany dies. If you play as a rogue/warrior, Carver dies.

Makes sense. Their role in the story would be a little redundant otherwise.
