One tile is hidden in Jaws of Hakkon, and two are still hidden in Trespasser, despite finishing both of them in my playthroughs.

For trespasser try re-doing the final moment where you talk to solas again. For Jaws the hidden tile relates to a quest chain that's not easy to notice.

One the tiles in Jaws of Hakkon requires you do to War Table Operation after speaking with Kenric about Ameridan's true nature (Dalish mage).

In Trespasser, the second tile is about the Inquisition's current status and the third tile is about Solas...
The first one is still hidden for me...

First tile is about Iron Bull. Did he stay loyal to you or to the Qun.
Bull stayed loyal to me and that tile is unlocked. Qun loyal Bull tile is bugged at the moment.

By the sounds of it I believe it will be Assuage Ameridan's Heirs, or if you lied to Kenric, Celebrate with Ameridan's Heirs - they're the operations that give you the schematics for the Dragon Hunter armours

Like bel3338 said. After you have met Ameridan go speak to Kenric about what you have learned. This unlocks an operation depending what you told to Kenric.

Spoiler tag doesn't work with DLCs yet. Are the unlocked tiles matching with your in-game decisions?

I've got an issue like this too.
Both Jaws of Hakkon and Trespasser are showing only one of the decisions among the possibilities and only about 2/3s (specifically in Trespasser) of them are the ones that I made. But the other option is still showing as Hidden and won't let me choose it to fix it to the correct one. I have Spoilers set to show but that doesn't explain why it's not showing some of the in-game decisions I made correctly.
I've tried re-playing the ending, I've given it a few days, logged in and out on console (Xbox One) and in the Keep, and played other characters. I understand that the Spoilers option may not work on DLCs right now, as was mentioned previously, but does that include a bug to not show some decisions correctly?
