What always confused me about it was, didn't the order know about red lyrium and what it did in Kirkwall at that point? Or was that all hush hush and blamed on blood magic? Or just specifically that idol?

Of course. I'd have no reason to distrust my commanding officers. The way it was written is that many just felt stronger at first, sharper in combat. So you wouldn't have known the down sides until it was too late most likely.
I cannot see me being a Templar, but if I was- I'd be in a military order. I'd take it.
From what we're told, it's not so much an offer as 'Here's your lyrium, recruit'. You may look at it and go, 'Sir, it's red, not blue?' and Sir would probably just be like 'Yes, but we get all different sorts of lyrium all the time, recruit, you should know this.' And then, as you are in a military organization where they have real methods of checking if you take said lyrium ('Can you smite this mage?'/'Uhhhhh...'), if you have any brains, you'd follow orders. Even if you're just following them till you get to a point where you can desert, you'll never get that option if your considered a troublemaker who can't even be trusted to take their stupid lyrium.
Now, if they were like 'Heyyyyy, want to try this new experimental lyrium stuff! It's coooool! Stronggg!', I'd be like, 'I'd rather not, sir'. But that doesn't seem to be what happened.
As everyone is saying, you don't really get to choose what your CO orders you to do. Although after seeing other Templars gaining new power after taking it, I would probably say hell yes where is mine.
No, considering I'd have half a brain and be intimately familiar with what lyrium is given I'd have been taking it for years.It wouldn't be the same as receiving and carrying out any other mundane order from higher command, because it would involve ingesting a substance I'd never seen that is very obviously different, of which the regular version already has severe enough side-effects on its own. My sense of self-preservation is too prominent.
