I picked up all 12 mosaic pieces in Western Approach.  I used a map and marked them all off.  My collections say I only have 11.  So I went to youtube and found vids on where to find each one.  With each of the videos I went to each place in game that the youtube vids took me,  I confirmed I'd picked up all 12 at those locations.  Yet my collections show only 11 of 12, and the piece is not on the wall.

 

Anyone else have this issue?  I know some will say that I must have missed finding one and miscounted or something, but I've gone through with my map twice, and step by step with the youtube video showing exact location to each.  I have, indeed, been to each place and picked up a mosaic at those locations.  

 

Still at 11 of 12.  Why?

The game trolled you. You might not like this, but the only solution i can think of is to start a new play through.

I know some of the Hinterlands mosaic's got moved around by a patch. If the same happened in the Approach, the guides you are following may be outdated.

Errg.  Well thanks for the reply.  The maps I use updated to the Hinterlands move and I have all them.  It's frustrating because I picked up a mosaic at every location  on that map and every place the youtube guy rode his horse to.  

 

Okay well, I expect I'll have another play through again after I start back at DA1.  I've been in the mood to play the whole thing over again.

And according to this wiki http://dragonage.wik...hdemon_(Mosaic) I am missing #8, which is in the cave with the old well under Griffon Keep.  Dammit, I DID get that one.  The completionist in me wants it on the wall!

SQUEE!  I went down to that well A FOURTH TIME (after a few logging outs and back in) and it showed up again in the cave and I was able to grab it and get credit for it! Problem solved!
