Leliana is good but she's not perfect, any organisation is going to be infiltrated at some point, and the larger it is the faster that infiltration is going to occur. Lets also look at the intense scrutiny new Agents are put under before being recruited 'Hey wanna work for us, great as the Inquisitor your hired' is pretty much the extent of a lot of those recruitments.
The bigger issue through is as mentioned not only is Solas an unknown threat at the start of Trespasser, but he knows the people in charge, knows how they operate and think.Which leaves the Qunari, again it's a growing organisation taking people from wherever, including Krikwall, which is a natural thing, after all Varric's from Krikwall, he has contacts there and is fairly well known, why wouldn't people from Kirkwall show up, and until they get activated it's likely the Qunari spies acted pretty much like anyone else.
If the Inquisition was still the small group at Haven sure Leliana could maybe keep most spies out, but by the time it's an organisation with three keeps on top of the Headquarters, and an army to match, well.... keeping it clean of infiltration is not just hard, it's outright impossible.

The requirement for spymaster is usually skin tight leather outfits.

You can do that and keep the inquisition. Remember . You're getting new feild leads. I keep the inquisition around because I KNOW the south is going to be kept in check and i want the flexibility.

Yeah but the hint at corruption makes me feel a bit uneasy. Oh sure we have more power to fight Solas, but at what cost? I think after being fooled by both Blackwall and Solas (and Qun Bull), my Inquisitor wants to stop, take a step back and slow things down. Maybe we can grow another type of organization in DA4 with our Tevinter allies? That would make for some interesting gameplay!
I intend to play both versions to see what's best when we get the next game, as well as choose to redeem Solas in one and kill him in another. I'm curious to how it will play out.

My canon Leliana was Divine and raising a nug army inside the Grand Cathedral. Priorities!
I'm not shocked she missed the spies. Her mindset was blown when the Divine died. Let's be honest, from the time the game started until she had her personal quest, she was no where NEAR the top of her game. Maybe middle, if we were lucky. She was strung out, heart broken, and lashing out. She was NOT making good calls, which is why on a few occasions she deferred to the Inquisitor (My Carta and Qunari Merc Inquisitors...eh, maybe might know what they were doing, but my Dalish and Human Noble Mage PROBABLY shouldn't be making those calls just yet). If not for Charter and the others, she might have totally screwed the Inquisition over (lets be honest, having a GREAT support team has allowed many leaders to coast through bad times). Her world had been demolished, so it wasn't shocking. By the time she WAS clear headed, Solas already knew how she operated, and the Qun was firmly entrenched. I'm certain a FEW spies were caught, but the bulk was so deep that nobody was sussing them out.
Well, except Solas...but he wasn't exactly volunteering his info, now was he? Time to join the Pack, Curly.
Still, she did the best she could with what she had, and she'll learn from it. Also, with a Romanced Warden back, she'll have a lot less stress on her hands...even IF she's Divine.
By the way, by the way...what IDIOTS though it'd be a good idea to try and kill the Divine who was buds with the Hero of Ferelden AND the Herald of Andraste? REALLY? Even one handed, the Inquisitor is a powerful figure (with the exception of an archer, they are still all basically Grim Reaper understudies...and the archer just needs to switch to a smaller crossbow). My Warden was an Arcane Warrior who was half in the Fade and half here. They've killed ARMIES by that point...armies of DARKSPAWN...villagers with torches and pitchforks is SUCH a step down...it's like a chainsaw through butter...

Friend, pretty much everyone from the Inquisitor's inner circle was a spy of sorts.
What do you think Varric was doing following you around? He has contacts and he spies for them. Do you think he stayed in Skyhold because he liked it? listen to his conversation with Hawke in the corridor leading to the War Room. He hates that Skyhold is in the arse end of nowhere and feels disgusted that 'people eat snails' there.
Sera was spying for the Red Jenny, pretty much an organisation formed by merchants, workers and other social classes who are being screwed somehow by the Orlesian nobility. This is why she was evasive when you asked her who was TRULY in charge of the group. Maybe she doesn't eve know for sure, but she does know SOMETHING. Regardless, she's faithful to THEM, the Red jenny, and therefore won't endanger them by telling the Inquisitor more than he or she should know. *wink wink*
Vivienne never lost her contacts with the Circle. The Inquisition decided the fate of mages all over Thedas. And you can rest assured the First Enchanters heard about the Inquisitor's actions from her the moment you finished the Champions of the Just or the In Hushed Whispers mission.
Dorian had friends in Tevinter - never mind his whole 'I'm a pariah' speech - who readily asked for the Inquisitor's help when they realised it was a rising power in Thedas. Willingly or no, he attracted the attention of the Magisterium upon YOUR institution once he involved you in Tevinter affairs. The Imperium might become a huge headache for the Inquisition in the future because of this. My guess is that the Lucerni will also begin to ask favours of the Inquisition in the years to come, leaving the Inquisitor knee deep in their deadly politics.
Why do you think Leliana let them stay in the Inquisition nonetheless? Because it's normal for any institution to do so. Institutions are not oysters; you don't shun people from it just because they are aligned with certain interests. Quite the contrary; you let them stay and see how your institution works, so that they may assuage the fears and insecurities other institutions - governments, companies, etc - may have of you. Otherwise, you become a mystery, an untrustworthy and unpredictable element in politics, a liability that sooner or later will be brought down by stronger forces who are watching you.
Leliana had those spies followed, since she keeps tabs on EVERYONE. In return, she also spied them, AND THEY KNEW. This is normal and I bet she used them to send specific messages to the organisations those spies served. And vice versa.
This is yet another clever move from Bioware. If you don't pay attention, you miss a lot of good stuff they insert in the games. If any company knows how to make fantasy mirror real life, it's them.
The only one that wasn't spying for others were Blackwall and Solas. But then again, after Trespasser, we know who the elf was spying for in the end: himself.
To spy and to be spied. That is the nature of institutions.
