I very rarely see one in conversations, and I see many, many listed in the wikia that do not appear.

There's a max to approval, once you reach that you can only get disapproval.

 

Are you sure you're not best friends forever with your companions?

Maybe that's it. I've done most of their companion quests. I can't seem to unlock Solas' though.

Possible spoilers, so:

If it's "All New, Faded for Her", then Solas will usually pop that quest after one or two conversations in Skyhold.

 

If it's the one involving elven artifacts, then it might not show up until you pop something like ten of them (with or without Solas present). You'll then get a wartable mission.

I'm not getting the quest from Solas either. My approval with him should be enough for it to trigger but at this point I've given up on it. I'll just glare at him every time I go to visit my BFF
