The book was The Calling by David Gaider. She found it in the deep roads while she was looking for the eluvian, she just thought to pass it on.
Morrigan's "lead" to help the HoF cure the calling

Because they either didn't know or had several ideas of what this lead could be. Unlike Mass Effect there is no clear start and finish, they seem (to some degree) be making stuff up as they go, thats not to say that they don't have a board overall storyline, it just feels like each game changes something of the previous game such as retcon's to the epilodge slides of Awkenings / Orgins and the messy timeline of DA2.
It's to big of a story to tie up in codex's and behind the scenes, we know that it has something to do with going west, beyond the western approuch / Hissing wastes and out of Thedas (i think) this means that the warden would be the first person in Thedas recorded history to leave Thedas and make it back without dying or getting lost in mysterious circumstances.
Personally I always assumed it was some free thinking awakened darkspawn who happened to know something and together they would travel to find this cure. Or Morrigan put the warden back in touch with the arcitect if you sided with him, It makes me laugh to think the HOF and arcitect are travel buddies.
Also a note as to why it makes sense for the HOF / OWC to be the one to find the cure:
The HOF / Orlesian Warden Commander searching for the cure makes sense, they know that the architect (who is a magister like Cory, it has been confirmed but we don't know if he has the same body hop ability as Cory, but I suspect he does) is also searching for some way to free the darkspawn from the call of the Old gods / Archdemons. If you side with the Architect i suspect the HOF / OWC work with him to find this cure as it affects both Darkspawn, Wardens, Red Lyrium and anything affected by the taint. Another reaosn why it makes sense for the HOF finding the cure is because Morrigan has this information, this lead and what other grey warden would she trust with such information? While morrigan only states she gave the HOF the information about the lead if your HOF romanced her, I suspect its the same for all HOF's.
