It MIGHT have something to do with their specialist class skill. I have the achievement and all of my squaddies evolved their specialist class into the one that gives more health/power duration.

Not that matters though because the second the Husks came in range I flipped on my Incendiary Ammo and sent tons of bullets down range. I had an 80 round clip Machine Gun. With the accuracy upgrade it wasn't pinpoint however it made holding down the trigger easier.
I did tactical rushing as I like to call it. I had my team give me long range fire support and then I storm vaulted the barricades got around the back and gave the heavies something to worry about. Worked for me all the time because my Adrenaline Rush was set to reduce my damage taken to health by 50%. I only got nervous when they did those rediculous Husk rushes. It's hard to stay behind cover when they are knocking you around.

I had all the upgrades from the team members, but didn't have all weapons and armors maxed out (assault riffle was 4/6 for example).

First off get all the major ship upgrades like the armor, shields, and weapons. Next, get everyone's personal upgrades if they have one and then do everyone's loyalty mission before doing the derelict reaper for the IFF. Once you get it, you can do one more mission before the crew is abducted if I recall so you can save that as legion's loyalty mission so you'll have all loyal squad mates.
I'm not sure about the other upgrades like weapons, biotics, and tech upgrades will up your chances, but it doesn't hurt your chances.
On the mission itself you gotta pick the right people for the job. The jobs are Tech specialist and fire team leader for the first part, the biotic specialist and fire team leader for the second part, have one person escort surviving crew/colonists, and then having the remaining people hold the line. If you don't pick the right person for the job, someone will die regardless of loyalty if I recall.
For tech specialist, Legion and Tali are your best choices. Everyone else just doesn't have high enough tech skills to close the door fast enough.
For both team leaders, Miranda, Garrus, and Jacob are your best bets for getting everyone through.
For the biotic specialist, Jack and Samara are your best choices. Everyone else just isn't as strong as a biotic compared to Jack and Samara.
For escorting the crew back to the normandy part, as long as it's someone loyal, anyone can do it but most people pick Mordin because he supposedly dies alot on the hold the line part (I never had that problem truth be told.).
For the holding the line part, I think that is based on a time limit as well. If you take too long then people start dying but for the most part it doesn't matter who you leave behind though some people have said that Mordin tends to die alot in that part so he's usually either brought along on the final boss or sent to escort the crew back to the Normandy.
