I've got a weird case where Samara's supposed to be dead according to the save editor, but she's showing up in the monastery anyway. I remember someone having that problem with Thane too, but I never heard of a solution. (This was based on an edited ME2 save.)
Edit: I can get around it by setting the flag for Shepard siding with Morinth and killing Samara on her LM. I get some weird Shepard dialogue and email from Morinth, but close enough.
Is there any savegame editor expertise around?

The game uses multiple flags for each scenario it seems, something similar happened to me with Tali where i missed a chance to lock in the romance before having finished every part of the game (and so no way to set the romance), i edited the save and then proceeded into ME3 and Tali was then indeed in a romance with my Shepard buuuut she then went to Garrus before i hit earth...
