Good call. I didn't even connect the dots.
OP has an unhealthy obsession, it seems.

It's about time someone did.
Does she charge in gold or peti fours? (whatever those death cakes were.)

I've yet to see a razor in the entire world either! How do the women keep their legs and pits hairless? How do the men keep their bodies hairless? They just didn't add those details.

Enchanted clothes that scrub away all grime, dirt, bacteria, etc. while working. This is also how all those blood stains suddenly disappear.
All Inquisition members are outfitted with such clothes, for efficiency and such. However, baths are still relaxing and Josie likes a more upper class lifestyle.

Well in comparable medieval times they just didn't shave often, wash often or even use sanitary pads. Even brassieres are a modern innovation, in fact toilet paper was not used back then.

I was lead to believe they used their fingers and a bunch of herbs mashed together.

What do women use for their monthly period? How do they dispose of it?
Do they use rags like the old school of our world did? Stuffed nugs? Blood magic?
Where are the "sanitary napkin disposal units" in Skyhold?

Have fun immersing yourself in those romance scenes then.
Solas & Blackwall have been on the road for a long time, and Sera does not strike me as particularly keen on, uh, social conventions.

She charges everyone to use the bath. It's her side business.
After all, the Montilyet tame the waves everywhere
