I need to ask this because after playing through the game several times I am 100% positive - as in I could bet my left kidney - that whoever killed Mythal controls the blight. Was it ever revealed or hinted at who killed Mythal?

Flemeth is absolutely furiously seeking revenge on who killed Mythal mentioning that "as long as the music plays we dance". The music here is (almost certain) her refering to the song. As in the song of the red lyrium controling the blight/darkspawn. 

 

Considering what evidence is at the table we know that Mythal killed a Titan, then was murdered at the temple of sorrows - probably by someone she trusted and possibly the catalyst to make Solas banish the elven "gods". The stone sings pure (living, untainted titan we visited in the decent) is one part of the coin. It is at this point safe to assume that the red lyrium is a dead, corrupted or controlled titan which song controls the blight. 

 

I may be abit off on some details here. But I am confident that the important part is that whoever killed Mythal controls the blight. Either by proxy (Elven god trapped in the fade using magic to control the blight), or in person as one of the elven "gods" are actually not banished and lives inside the corrupted titan in the physical world. Note: Didnt finish trespasser yet. Might prove this theory wrong or strengthen it.

My theory is after Mythal killed a Titan and the Evanuris discovered the power of their blood (lyrium as we discovered in Descent) they unleashed the Blight while digging for more, and corrupted the lyrium. Mythal tried to stop them from continuing so they killed her. It's hinted at too in a few codex entries in Trespasser.

It seems that after mining the Titans something bad happened (presumably the Blight) and then they sealed the Titans under ground.

As for who killed Mythal, it seems that the other elven gods, excluding Solas and Forgotten Ones perhaps.

Yep! And Solas said that their lust for power drove them to kill her so I suspect they wanted more lyrium when they realized what it did to their magic. That one codex were they are desecrating the Titan's body with slaves building a statue from it for Elgar'nan was revolting. Also this was the same time when Solas finally turned against the Gods too, and he told us it was Mythal's death that pushed him, yet it was also at the same time they had sealed the Deep Roads.
