DA:O - Hero
For the hero I wish to use, two files pop up for her. Is one just considered a different save file because of the expansion?
Yeah, there is a save from the basegame and from Awakening/Witch Hunt.
I wish to use my hero's appearance, but wish to change her class to Rogue (I picked warrior but ended up playing as a dual wielding Rogue). How can I do that?
You can upload a custom portrait to any character. Just choose one of the default heroes and replace it's portrait.
DA:O - Companions
For acquiring the grimoire, what difference does it make to either get it peacefully or to get it by killing Flemeth?
As far as I'm aware, none. Peacful means mean you do not kill her, but since she gets revived in DA2, it gets retconned anyway.
Is there any difference as to who executes Loghain?
As far as I'm aware, none. It might be for roleplaying purposes, as your character aspires to be king/queen or Alistair has the same ambitions for taking the throne and securing his country.
DA:O - Awakening Expansion
The Architect, what was his deal, and should I let him live or not?
He wants to disconnect the darkspawn from the Taint and freeing them from the Old Gods' call, thus preventing future Blights. He puts the darkspawn through a reverse-joining with Grey Warden blood, for which he abducted, killed and experimented on Wardens. He tried the reverse joining on a sleeping old god, but ended up corrupting him and starting a new Blight. Nevertheless, he intends to perfect his methods and continue the work. Letting him live depends on you - if you trust him after all he has done, and think of the reverse-joining as an alternative means to end blights (the main duty of the Wardens is to end blights), then you may support him. Or, if you think he's gone too far and is too dangerous to let him live, then kill him. Oh, and he's one of the seven magisters who went to the fade and became the first darkspawn.
DA:O - Witch Hunt
Yeah, never played this one. Care to explain each of the three choices? (destroy the mirror, go through the mirror, stab Morrigan)
No destroying the mirror, instead, you can let Morrigan go through alone. Whatever happens, she survives and reappears in Inquistion. If she has a child and you go with her, then she will mention that for a few years, they all lived together as a happy family.

DA2 - Companions
Who should I romance, Isabella, Anders, or Sebastian? What effect (without major spoilers) does this have in DAI?
No effect in DAI, as none of the characters appear. Hawke and Varric will have a few lines about the chosen love interest, but that's it.
Which is better, to have Bethany join the Circle, or the Grey Wardens?
Doesn't matter. Grey Wardens will play a large role, but no Bethany except Hawke and Varric mentioning her.
Bartrand, better off dead or alive?
Varric tells the story of the red lyrium idol, there he mentions his brother's fate, but other than this, nothing else.
Anders, execute him, or let him live?
If he lives, you can hear about him trying to help the mages, but they resent him for what he's done. He does not appear otherwise.
Let Merrill destroy the mirror or keep it?
If you make destroy it, she will realise blood magic is not so good as she first thought.
Tallis, better off on good terms or angry?
It might affect Hawke's relation with the qunari, but no effect in DAI.
DA2 - Prologue
What's the difference between joining the mercenaries and the smugglers?
Other than a few different quests in DA2, no effect in DAI.
DA2 - Act 1
Kill Kelder?
He's a murderer and a pervert (she kills only beautiful girls), but his father wants to bail him out. If you do not kill him, he will continue killing. No effect in future titles.
Kill Ginnis?
You must kill her if you decide to rescue the Viscount's son as she will try to take him from you. No effect in future titles.
Kill Danzig?
He's a slaver who sold the mage boy to others, depends on you views on slavery if you kill him. No effect in future titles.
Tell the truth about the man's wife's death, or spare him the details?
Depends on your character personality. What whould s/he do? No effect in future titles.
Send the elf to the Circle or the Dalish?
Depends on your standing in the mage-templar conflict. No effect in future titles. He will go to Tevinter, if you save him again.
Defend Ketojan?
Depends on your standing in the qunari business. Do you agree with how the qunari treat the mages? And do you want to help the sister frame the qunari in the eyes of Kirkwall's peope? No effect in future titles.
Kill Karras?
Depends on you approach in the business of the escaped mages. If you fight against the templar reinforcements and let the mages go free, you have to kill him. No effect in future titles.
DA2 - Act 2
Kill Gascard?
He was an apprentice to the man who kills Hawke's mother. If spared, he can perform a blood magic ritual to track Hawke's missing mom, but will betray you for his old master and has to be killed. No effect in future titles.
Side with Varnell?
He's part of the radical chantry group that wants to antagonize the qunari. He abducts and kills some qunari, and you can choose to support him or free the qunari. No effect in future titles.
Side with Petrice?
He's with the same group as Petrice, so following her leads to the same - try to frame the qunari. No effect in future titles.
Yevhen and his sons?
If you didn't recurit Nathatinel in Awakening (or he died) you will meet Yevhen, whose sons are missing in the Deep Roads. You have to choose who to save. No effect in future titles.
Let Javaris die?
Javaris was set up by the qunari with false information, they let him steal a powder formula. Javaris - believing it to be an explosive weapon - creates a poison gas that infects a district. The arishok will judge him and leaves his fate to Hawke, who will have to decide if his crime was grave enough to be punished by death.
DA2 - Act 3
Help Nuncio?
He's after Zevran, but he does not tell you this, you find out later. If you don't help him, no Zevran in DA2.
Who to hand over the conspirators to; the mage or the templar?
Depends on which side do you support. The knight commander will have confirmation that the mages are growing out of control, and the first enchanter will use it to try and not make things worse for mages. No effect in future titles.
How to help the uncle?
If you do the quest, you can reunite him with his long lost daugher (Hawke's cousin) or get him the false gem that he can sell. No effect in future titles.
What to do with Emile?
He escaped from the Circle. He can be taken back or let go free. He's a harmless and naive mage. Depends on your views on mage liberty. No effect in future titles.
What to do with Keran?
He was with the conspirators who abduct your loved one/friend/sister/brother to make you help them. Depends on how you view such an act. No effect in future titles.
Kill the dragon?
This is Dragon Age. Of course you kill the dragon.
DA2 - Legacy
Who to side with in the prison?
Janeka wants to control Corypheus for her own uses, Larius wants to kill him for he is to dangerous and cannot be controlled. In the end, they both say Corypheus has to die. The choice in mentioned in DAI, but no different outcome.
