When is it really OK to play Gold?

The best game I have played on Gold was a team of two on Hazard Reactor - I was QFE, my colleague was a Volus Adept IIRC. We managed to funnel the majority of the Geth (we wouldn't have won if it was anything else) through the reactor while alternating between blasting them with powers and toasting them in the reactor. I was stunned when we actually made it.
That was a highlight. The lowlights mostly involve me being synched repeatedly, getting pwned by ninja Engineers placing turrets behind me, and fumbling away chances to carry a team. Should I really be playing on Gold? I don't want to waste players' time but Silver is pretty easy for me at this point. Are there any other factors I should consider in deciding whether to play Silver or Gold?

Real players don't talk about gold - they play it and squeeze it hard with both hands.

If you feel that silver is too easy, then you should be ready for gold. But how is your manifest ? Using rare and ultra rare guns is advised for gold, as well as level 2-3 ammo and weapons amps. Some characters are also easier to use than others for first steps on gold.

However if you play anything else (especially lamer, snap cheese, recon mine spammer or venom) I'll complain and probably kickvote you or ragequit the lobby to look for quarians if that doesn't work.

Also, if you have an Avenger 10 and a Human Biotic or Engineer, you're gold ready, at least in terms of manifest. In terms of skill...well, that's for you to decide. But if you're finding Silver boring, I would suggest taking those two classes, since you definitely have them and they're real powerhouses with good CC/AoE which don't require good weapons to function well on Gold, and use them to build up your manifest.

As I've mentioned, I regularly play and extract on Gold but nearly always finish last and often have to be revived. Yet I like to think I assist the team.

In gold, situational awareness and spawn knowledge is key. Rule of thumb is that enemis spawn is the furthest corners (or closest spawn to each objective) from the players.Also, weapon choice and equipment. You don't need to go out blasting with a Harrier, just how to maximize each weapon for a given character. Your QFE for example is best served with a Shotgun/Talon with decent range such as the Eviscerator or Piranha. With disruptor ammo, you get Cryo Explosion, Fire Explosion, and Tech Burst.Also, never camp. If you stay in one place too long, you'll eventually be overrun. In a place like reactor, the top is a great spot to hold down, but beaware of your flank; the left side is usually safe as an escape plan. Never stay in the LZ of any map on extraction and know that each enemy faction has a weakness. (Reapers->biotics, Geth->Tech, Cerberus->full bio/tech team, Collectors-> Armor Piercing). Be aware of Right Hand Advantage (using corners on left for cover while shooting) and utilize grabs over cover whenever possible. Lastly, try to conserve your missiles for big spawns such as reaper wave 8 or clearing the way for objectives; do not save them for extraction wave unless you have no reason to nuke beforehand.

If you regularily play and extract on Gold, you're OK. Being last on the scoreboard means little to nothing. As long as you're not a crippling liability and getting votekicked out the lobbies all the time, you evidently do well enough.
All that's left is getting better at it.
As was already mentioned, map knowledge should already be a thing, spawn knowledge is not exactly a necessity to survive but helps IMMENSELY (and becomes a necessity if you want to do really good, hunting topscores)plus the usual yadda yadda about using leveled up kits, powerful weapons and equipment/ammo boosters blah blah.
As time and mastery will come you'll find the former two being ingrained (I haven't played ME3MP for what, two or maybe three years now but I still know the spawn patterns, they are easy and predictable once you figured the basics out) and yourself scoffing at the latter three (lvl 1 kits with common/uncommon weapons just for the lolz, failed votekick and then topscore to rub it in).
And in the end always remember it's not about getting high scores or about extracting, it's about having fun. And if having fun for you means strolling around with a Predator on a Volus Vanguard in Gold, then that's how you roll (heh, roll ...).

If you're doing objectives and using medigel when appropriate and you usually make it to extraction with a pug team more often than not, you're ready for it.

As soon as you have enough armor piercing (mods or ammo) to regularly fight bosses.
Other than that, just try to be a good team player and keep moving. Moving around is the most important thing as this helps you from being flanked. Just because you are shooting at something doesn't mean you have to kill it before moving to another location (at the cost of many med gels / ops packs and missiles I learned this)

Stick to mostly silver with a trip to gold every now and then. When you no longer feel the need to ask this question then it's time to focus on gold.

If u have a feeling of guilt when others carry you (or pride makes you want to keep up), drank your vitamins(loaded consumables), you probably are long ready 4 gold. The rest is irrelevant.
Playing without high level consumables on Gold and with Pugs that don't use them was most of my pugging experiences back in the day. And can honestly say it works very well for Gold.
So don't worry about your consumables, OP.

OP, you're also forgetting that being at the bottom of the scoreboard also means the players above you are all very happy they aren't last.

Good point. I'd add that the BSN sweet spot is playing well enough that the BSN'er can kill things without having to check his flank, but not playing so well as to threaten his score.

You could use the kits you are best with, and search the map/enemie combination you are most comfortable with. As a starter.
Other then that, help with objectives, use gels and nukes when they are needed, and it`s all good
