Does Vivienne know to charge up spirit blade to +90 before using it? I know they made a change with spirit blade not sure she know when to use it?

No. Just give her the new upgrade (that builds charge more quickly) and leave her alone. She'll still spam it when she decides to use it, but at least she'll probably have full charge for the first strike.

How do you even get her to use the spirit blade? She never uses it unless I play as her and I don't want to play as her.

Well I guess you can set her to follow; either herself or the party tank, in the tactics menue; She needs to get in melee range. You can also tinker with the spell preferences.

Or turn it off in tactics and just switch back occasionally to handle it manually.

Or do as I do to her, make her a spirit mage, she has very little besides KE to attack with that way.

chain lightning and energy barrage helps too to recharge her blade (and draw aggro) when set to preferred, but sucks obviously in friendly fire setup.

It needs to charge up to +90 now and she doesn't know that. If she uses it all the time it will do very little damage as of the last patch.
Best to disable it and run it manually for her.

I did that and disabled spirit blade run it manually. The best possible outcome.

Someone asked how to get her to use it. If you are not interested in micromanagement, that is the easiest way to get her to use the skill.

Actually, the best setting to do that is to set her to Defend the tank (she will stay close, but select her own targets.) Follow self can sometimes mean she stays at range a bit too much with auto attacks etc
