I've started taking Iron Bull along on dragon slaying expeditions to try to get his approval up. The wiki says you get Greatly Approves when you kill a dragon and he's in the party. But I'm only getting Slightly not Greatly.
I'm wondering whether he needs to actually attack the dragon before it dies? That hasn't happened so far (I use the Assassin/Tempest method, so the dragons die before Iron Bull can get near them). If it isn't that, I don't know what it can be. Is the wiki wrong?

When Iron Bull is in your party and you kill a dragon Bull will ALWAYS give you, "Greatly Approves."
However there is an exception, if your character has high enough approval already with Bull, you will gain "Iron Bull Approves, "Iron Bull Slightly Approves " or no approval.

Yep. The approval ceiling is 125 points. "Slightly" adds 1 point, "Approves" adds 5 points, and "Greatly" adds 20 points. With 12 optional dragons in the main game, that gives an option of 240 points from Bull, well past the maximum.
