I never got one. I assumed it would come with the game. Who/What should I contact to remedy this?

I ordered over Amazon, if that makes a difference.

EA gave me an automated response about the Blood Dragon Armor when I asked through them. Is there any other way to do this?

Amazon sends its preorder codes out over email a couple of days after the game releases. I think they actually say it can take up to 10 days, but I've never seen it take that long.
